Your Impact
As a Hospice RN Case Manager you will ensure comfort and compassion guide every interaction. Your responsibilities include:
Holistic Patient Care: Perform physical emotional and environmental assessments to create individualized care plans.
Meaningful Visits: Deliver hands-on nursing care ensuring every interaction supports the patients well-being.
Team Collaboration: Communicate vital updates to supervisors physicians and care team members to align care strategies.
Empowering Families: Educate and support loved ones offering guidance and resources to navigate their journey.
Proactive Response: Monitor and address changes in condition promptly ensuring the patients care needs are always met.
Commitment to Excellence: Follow all care plans and physician directives delivering care that prioritizes dignity and quality.
What Youll Need
Graduation from an accredited nursing program (Certificate Diploma Associate or Bachelors).
Current RN license in Michigan.
A minimum of 1 year of clinical nursing experience
1 year of Hospice or Home Health Experience is REQUIRED
Reliable transportation for patient visits.
Ability to sit stand bend lift and move intermittently with the capability to lift up to 50 lbs.
A dedication to multitasking documentation and meeting deadlines while delivering outstanding care.
