Job Description
Family Support Worker
- The incumbent will provide technical assistance to the Craniofacial interdisciplinary team regarding family engagement and patient advocacy initiatives both during weekly Craniofacial clinic and ongoing quality improvement initiatives.
- The incumbent will develop patient engagement programming including but not limited to a network of patient and family advocates to expand the patient and family voice improve clinic functioning and develop ongoing Craniofacial patient and family engagement programming.
- The incumbent will meet with patients and families to provide psychosocial support resource referrals and technical assistance at patient and provider direction.
- The incumbent will complete assigned duties and assignments as needed.
$20.00- $25.00 per hour
High school diploma or equivalent and one year of experience working with children or families.
2 years of lived experience as a parent patient or family member of a child/youth with a special healthcare need.
