Job Description

Job Summary:

The Special Education Transition Coordinator/Special Education Teacher is responsible for developing implementing and coordinating transition services for high school students with disabilities. This includes assisting students in the transition from high school to post-secondary education employment and independent living as outlined in their Individualized Education Programs (IEPs). The coordinator works closely with students families school staff community agencies and post-secondary institutions to ensure a smooth and successful transition process. They will also have a caseload of students they support directly.

Key Responsibilities:

  • Develop and coordinate individualized transition plans for students with disabilities ages 1421 as part of their IEP.

  • Collaborate with special education teachers general education staff counselors families and outside agencies to support transition goals.

  • Facilitate transition assessments to help determine students strengths interests and needs.

  • Assist students in exploring and securing post-secondary options including vocational training employment college programs and community resources.

  • Provide direct instruction or support in areas such as job readiness independent living skills and self-advocacy. Visit students at their work sites providing feedback and collaborating with the site supervisor to ensure student success in these experiences.
  • Coordinate and train/support the job coaches assigned to students. This includes supporting the job coaches in completing student work evaluations to support their post transition skill development.
  • Across PGHS support and attend student IEP meetings to discuss transition support services and options for students to develop student transition grids.
  • Coordinate work-based learning experiences internships job shadowing and job placements for eligible students.
  • Coordinate transportation for all students to and from work sites.

  • Build and maintain partnerships with local businesses community agencies colleges and vocational programs. This includes securing partnerships with jobs and other businesses in the local community as we develop work placement opportunities for students.

  • Monitor student progress toward transition goals and document activities and services in accordance with IEPs and compliance standards.

  • Stay current on state and federal regulations related to transition planning and special education.

  • Provide professional development and support to staff and families on transition-related topics.
  • Maintain positive and collaborative relationships with the Office of Rehabilitation (OVR). Host informational sessions in collaboration with OVR for families.
  • Engage and work collaboratively with related service providers to ensure that accommodations and assistive technologies needed in obtaining employment are provided within the transition process.
  • Other jobs as assigned by the Special Education Supervisor Building Principal or Director of Pupil Services.
  • Develop and coordinate instructional assistant absences at PGHS daily in coordination with the Special Education Supervisor and PGHS Assistant Principal.

Qualifications:

Education & Certification:

  • Bachelors degree in Special Education or related field (Masters degree preferred)

  • Valid teaching certificate with a special education endorsement

Experience:

  • Minimum of 3 years of experience in special education or related transition services

  • Familiarity with IDEA IEP process and post-secondary transition planning

  • Experience working with community agencies employers and post-secondary institutions is a plus

Skills:

  • Strong communication collaboration and organizational skills

  • Ability to work effectively with diverse populations and teams

  • Knowledge of transition assessments career exploration tools and assistive technology

  • Commitment to student-centered practices and inclusive education
