What you bring to SBHG (Qualifications):
- A masters degree in social science is required plus one (1) year of experience working in behavioral health.
Or-
- A Bachelors degree in social science is required plus (2) years of experience working in a behavioral health field.
Or-
- Associates degree in social science required plus (3) years of experience working in a behavioral health field.
- Valid California Drivers License
- Meets Agency driver eligibility criteria
How you will make a difference (Job Overview):
This position coordinates the care and serves as ECM Point of Contact/Lead Care Manager for the Full Circle Health ECM qualifying youth.
Division/Program Overview:
An ECM Manager is a Lead Care Manager specializing in Enhanced Care Management providing comprehensive support to enrolled individuals. They act as a central point of coordination connecting members with vital resources such as housing food and childcare while also collaborating with diverse healthcare providers to ensure seamless and coordinated care.
