Education: Graduate from an accredited college of pharmacy (minimum B.S. degree in Pharmacy)
License: Current valid unrestricted pharmacy license in any U.S. state
Experience: Minimum of 12 months as a practicing pharmacist within the last 36 months
Dispense compound and prepare medications including IV additives in both inpatient and outpatient settings
Provide counseling to patients on proper use of medications and ensure adherence
Review prescriptions and medication orders for appropriateness allergies drug interactions and therapeutic duplications
Collaborate with healthcare providers to resolve medication-related issues and optimize therapy
Participate in clinical rounds and provide specialized pharmaceutical care when needed
Maintain proper documentation of medication distribution and controlled substances (including Schedule II drugs)
Review and verify MAR (Medication Administration Records) and discharge medications
Comply with JCAHO HCFA and hospital standards and procedures
Monitor inventory levels and manage emergency medication supplies
Document all pharmacy services and interactions in the Electronic Health Record system
