Job Summary:
We are seeking a compassionate and skilled Physical Therapy Assistant (PTA) to join our home health team. Under the supervision of a licensed Physical Therapist the PTA will provide direct patient care in home settings assisting patients with therapeutic exercises and interventions to enhance mobility relieve pain and achieve rehabilitation goals. This role requires strong interpersonal skills adherence to HIPAA regulations and a commitment to delivering high-quality care in compliance with the Patient-Driven Groupings Model (PDGM).
Key Responsibilities:
- Deliver physical therapy services in patients homes as directed by the supervising Physical Therapist and in accordance with the physician-approved plan of care.
- Assist patients with active and passive therapeutic exercises muscle re-education functional training transfer activities and prosthetic training.
- Instruct patients and caregivers on proper use of assistive devices (e.g. wheelchairs crutches canes) and home safety techniques.
- Document patient progress treatments and outcomes accurately in electronic health records (EHRs) adhering to HIPAA compliance standards and PDGM documentation requirements.
- Monitor patient responses to therapy reporting any concerns or changes to the supervising Physical Therapist.
- Maintain a clean and safe environment for patient care following infection control protocols and ensuring proper handling of equipment.
Qualifications:
- Associates degree from an accredited Physical Therapy Assistant program.
- Current PTA license in State (or eligibility for licensure).
- Minimum 1 year of experience in home health outpatient or skilled nursing facility settings preferred; new graduates may be considered with strong clinical internships.
- Valid drivers license reliable transportation and auto insurance (required for home visits).
- Current CPR certification (AHA preferred).
- Familiarity with electronic medical records (EMR) software and HIPAA compliance.
- Knowledge of PDGM requirements including OASIS documentation is a plus.
- Strong interpersonal communication and time-management skills.
- Physical stamina to perform therapy tasks and assist patients with mobility.
- Ability to work independently in diverse home environments while maintaining professionalism and safety.

HIPAA Compliance:
- The PTA will adhere to HIPAA regulations ensuring the confidentiality and security of protected health information (PHI) during home visits documentation and communication.
- Completion of HIPAA training and signing an Employee HIPAA Compliance Acknowledgement are required upon hire.
