Must have the ability to travel to various locations to meet with partners (internal and external) on an as-needed basis.

General Purposes of Job
The goal of this role is ensure DaVita is the provider of choice to hospitals. This teammate will engage renal discharge planners and case managers in an effort to build and strengthen relationships. Their focus is to ensure a smooth and seamless experience in support of newly diagnosed and hospitalized dialysis patients to ensure a successful transition to the outpatient setting.

Essential Duties

  • Consistent outreach to discharge planners to provide education and build awareness of offerings
  • Identify opportunities to grow as provider of choice through relationship building and addressing feedback
  • Partner with internal field teammates to optimize the admission process and implement best demonstrated practices
  • Assist with necessary communication between hospital admission and clinic teammates to work through patient placements with extenuating circumstances
  • Support a smooth and seamless discharge experience for patients and hospital staff to DaVita centers
  • Develop comprehensive knowledge of local markets

Minimum qualifications

  • Bachelors Degree preferred
  • Ability to travel to various locations to meet with partners (internal and external) on an as-needed basis. Includes valid drivers license in state of residence and insurable driving record
  • Intermediate computer skills with a variety of software packages including but not limited to Microsoft Word Excel PowerPoint billing systems

Salary/ Wage Range

$69000.00-$117000.00 / year

If a candidate is hired they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position. New York Exempt: New York City and Long Island: $64350.00/year Nassau Suffolk and Westchester counties: $64350.00/year Remainder of New York state: $60405.80/year New York Non-exempt: New York City and Long Island: $16.50/hour Nassau Suffolk and Westchester counties: $16.50/hour Remainder of New York state: $15.50/hour Washington Exempt: $77968.80/year Washington Non-exempt: Bellingham: $17.66/hour Burien: $21.16/hour Unincorporated King County: $20.29/hour Renton: $20.90/hour Seattle: $20.76/hour Tukwila: $21.10/hour Remainder of Washington state: $16.66/hour
