PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Health-PSS team as the Lead Case Manager at the Los Angeles Location.
ABOUT PATH
Since its foundation in 1984 PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California connecting our clients to a comprehensive continuum of homelessness prevention street outreach employment preparation and placement assistance individualized case management supportive services interim housing and permanent supportive housing.
ABOUT THE JOB
The Lead Case Manager Hospitals will work independently out of hospitals representing PATH and coordinating care for people using the emergency department who are also experiencing homelessness. The role will work alongside funder leadership and direct supervisors to provide quality care to participants including reconnection to programs and service linkages and will also be responsible for data collection attending funder meetings to share ongoing program updates and working with other team members to ensure the medical and housing stability of participants.
- Work independently with hospital staff to provide outreach to individuals experiencing homelessness and using the emergency department; outreach will include and is not limited to
providing linkage to services reconnection to services and enrollment in available programs - Work with hospital staff to develop strategies and provide trainings to increase awareness of the
program and use of referral system. - Track documentation internally and externally using spreadsheets HMIS CHAMP Vertical Change and other program reporting systems.
- Ensure all services and encounters are entered into data systems for each participant served on hospital grounds.
- Complete program specific assessments and data reports in preparation for meetings with funders.

PREFERRED QUALIFICATIONS
- Three (3) years of experience in healthcare case management homeless services or working with vulnerable populations.
MINIMUM QUALIFICATIONS
- One (1) year of experience in outreach healthcare case management homeless services or working with vulnerable populations
MANDATORY REQUIREMENTS
Regardless of qualifications candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role all candidates must:
- Have reliable transportation
- A valid drivers license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATHs insurance coverage
