What will you be doing
Responsible for the provision of case management and counseling services to clients who are diagnosed with persistent mental illness.
Work with a team of behavioral health service providers within the agency.
Attendance at scheduled work hours including evening hours
Provide effective individual couples & familycounseling as assigned
Complete clinical assessments that meet the requirements of program funders and applicable administrative rules
Refer clients for community-based case management services as needed
Complete required documentation (IMCANS documentation including encounters consents) on a timely basis
Meet minimum service provision/billing standards.
Acquire and maintain required certifications and trainings
Maintain professional judgment and performance regardless of personal issues
Provide outcome evaluation data to supervisor.
Assist client in maintaining public entitlements as applicable.
- Provide counseling/therapy services to clients in the setting of their choice (ie. Home office or community).

What will you bring to the table
Education: Bachelors degree in a human service field required
Experience: At least 1 year of experience in behavioral health preferred

Physical:
Navigation of stairs
No lifting restrictions
Ability to provide services to clients in their homes

Computer including Microsoft Windows applications

Phone
Additional Requirements:
The use of personal automobile a valid drivers license and the minimum amount of liability insurance as defined by AIDs Personnel Policy.
Must drive self and clients in personal or agency vehicles.
