Job Description

Care Manager (Grade 5 RN) Memory Support Unit

Location: Homestead Estate Wallington VIC

Hours: Monday to Friday 7:00 AM 3:00 PM (or variation depending on operational needs)

We are seeking a hands-on caring and passionate Care Manager (Grade 5 RN) to lead our Memory Support Unit at Homestead Estate in Wallington.

Located in the picturesque Bellarine Peninsula Homestead Estate offers modern aged care services within a stunning environment. Our residence boasts light-filled spaces landscaped gardens and views of the surrounding countryside creating a welcoming and comforting home.

At TLC Aged Care Wallington we provide a comprehensive range of on-site services including: Community medical centre & allied health services Hydrotherapy pool physiotherapy gym & dietetics Residents sports bar piano bar caf & virtual reality cinema

What Youll Be Doing

As a Care Manager you will provide exceptional dementia care working closely with a high-performing team to ensure a safe and supportive environment for residents. Your key responsibilities include:

  • Delivering comprehensive person-centred nursing care
  • Developing recommendation plans & behavior support strategies
  • Managing medication administration & wound care
  • Driving continuous clinical improvement for excellence in care
  • Supporting residents families and visitors with professionalism
  • Ensuring timely assessments care plans and reviews
  • Leading clinical education & staff competency training
  • Fostering a positive and collaborative workplace culture

What Matters to Us

Were looking for a dedicated professional with:

  • Minimum two years experience in a clinical leadership role
  • Strong understanding of Aged Care Quality Standards
  • Division 1 Nursing qualifications & AHPRA registration
  • Up-to-date COVID-19 & 2024 influenza vaccinations
  • NDIS Check (incl. Police Clearance)
  • Eligibility to work in Australia
