Job Description

Scheduled Weekly Hours:
40Work Shift:
Days (United States of America)Provide care coordination for program-eligible pregnant women to help ensure they have a medical home for prenatal and post-partum care
practice appropriate pregnancy self-care get connected to needed social services to remove barriers to care and learn basic maternal and baby care with the goal of achieving a full term healthy birth weight birth. Care coordinators educate clients on topics including safe sleep practices benefits of breastfeeding and dangers of tobacco use. Services are clinic and home-based.
Preferred: Certified Community Health Worker.
Required: High School diploma or GED valid Ohio Drivers License and reliable transportation. Should have good oral and written communication skills enjoy working with people be a self-starter have knowledge of community resources and have good computer skills.
