Overview:
We are seeking a boardcertified/boardeligible Hospice & Palliative Care Physician who can also provide inpatient Hospitalist coverage at a wellregarded hospital system in New Haven Connecticut. This fulltime permanent position offers a rewarding blend of continuityfocused palliative care and dynamic hospitalist practice within a supportive teambased environment.
Job Setting:
Work within a multidisciplinary palliative care team committed to providing comfortfocused holistic care for patients with advanced illness. Collaborate closely with hospitalists specialists nurses and case managers to optimize patient outcomes. The role includes scheduled hospitalist coverage and participation in familycentered goalsofcare discussions.

Key Responsibilities:
Provide direct palliative and hospice care in inpatient and consultative settings
Manage complex symptoms and coordinate care plans aligned with patients goals and values
Serve as a hospitalist during scheduled inpatient coverage shifts (rotational)
Collaborate with nursing case management and specialty teams for integrated care delivery
Participate in interdisciplinary team meetings and quality initiatives
Provide guidance and support to patients families and caregivers
Educate medical staff on best practices in palliative and endoflife care
Requirements:
MD degree from an accredited institution
Board Certified/Board Eligible in Internal Medicine or Family Medicine
Fellowship trained and/or Board Certified in Hospice and Palliative Medicine
Active or eligible Connecticut medical license
Experience in both palliative care and inpatient hospital medicine strongly preferred
Exceptional communication empathy and collaborative care skills
Commitment to improving quality of life for patients with serious illness
