Were on the lookout for a FullTime Social Services Assistant to join our warm residentfocused care team. Youll be an essential part of making our residents feel seen supported and heard throughout their stay.

What Youll Do:
Assist with admissions discharges and resident care plans
Support residents and families in navigating care transitions
Help coordinate appointments community resources and family communication
Advocate for residents rights dignity and emotional wellbeing
Maintain documentation in accordance with regulations and facility policies
Partner with the Social Services Director and interdisciplinary team to provide the best care possible
What Were Looking For:
A warm empathetic communicator with a teamoriented mindset
Organized and reliable with solid timemanagement skills
Previous experience in social services or healthcare setting is a plus
Strong desire to support residents emotional and social health
