The Rehabilitation Services Coordinator collaborates communicates and coordinates with all members of the patients Home Health interdisciplinary team. Their role is to assist the interdisciplinary team by scheduling patient visits obtaining physician orders and communicating changes to the plan of care. Works closely with nursing leadership to troubleshoot and resolve issues. Serves as a Home Health representative when engaging with patients their family members physicians and the community.
The Rehabilitation Services Coordinator performs all duties in a highlevel efficient and effective manner in accordance with departmental objectives assuring optimal services are provided to the highest level of productivity. The Home Health Rehabilitation Services Coordinator is primarily an administrative position.
The Rehabilitation Services Coordinator will serve as relief for the Rehabilitation Services Manager as needed. This position coordinates daily resource allocation across the departments and locations assuring all ordered treatments are completed participates in department program development and performance improvement activities. This position assists the Clinical Director in all assigned duties in provision of daily operational oversight of rehabilitation services for Home Health and Hospice agency and serves as an administrative resource for these departments.

Has ability to articulate to the community of constituents the value range and necessity of home health services both from acute care and longterm care perspectives.
Possesses requisite knowledge and skills in discrete therapy specialty areas as appropriate.
Strong computer skills with knowledge in Word Excel and Electronic Health Record (EHR) documentation
Very familiar and comfortable with interpretation of Title XXII and the Medicare Act.
Able to solve problems. Sets priorities integrates changes and organizes work activities in a logical and timely manner. Demonstrates a consistent level of performance and productivity.
Always strives to make good use of time seeks out work that needs to be done and reports free time to manager.
Responsible for completing all assigned procedures during shift without sacrificing the quality of work.
