The role will focus on supporting people with diabetes and encouraging engagement from patients getting patients in for their routine blood apts vaccines etc while offering a holistic mental health and wellbeing support service with a focus on safeguarding.
Integrated Neighbourhood Team (INT) Coordinators are funded to improve patient care and collaboration across the City East Oxford Primary Care Network (PCN) which includes three GP practices. Responsibilities include organising and attending daily MDT meetings to enhance patient care planning running weekly MDT sessions and coordinating with healthcare professionals to ensure followthrough on planned outcomes. This role involves supporting a range of community health initiatives focused on tackling inequalities promoting independence and fostering partnership.
Primary Care Wellbeing Workers take a personcentred approach in the delivery of 1to1 sessions for people who are experiencingmental health issues or other social or lifestyle issues that are impacting on their wellbeing. The support provided aims to encourage and enable people to link in with existing support services use the support available in their local community and develop tools to increase their ability to manage their own wellbeing. The service offered is based on a social prescribing model and the role works very closely with Primary Care as an embedded worker within GP Practices.
