The Role:
We are looking for an enthusiastic individual for the permanent position of Team Leader within the Watergate Primary Care Network.
With the advice supervision and support of a Senior Manager as Team Leader you will manage a statutory service to vulnerable adults providing a full range of statutory duties within a legal framework in line with legislation and policy.
The primary role of a Team Leader in Adult Social Care is to improve the welfare safety and outcomes of the vulnerable adults and their carers. You will lead a team to deliver the vision and ambition of a sustainable Adult Social Care to meet the demands of today and the future. As Team Leader you will directly manage and supervise social care staff to carry out person centred strengthbased assessments enabling people to remain at home building upon the support from family friends and wider community. Helping to ensure that casework decisions effectively safeguard adults promote their wellbeing and improve the quality of their lives.
As Team Leader you will drive and embed quality and consistency of practice using reflective supervision as a mechanism to coach and support the workforce. Responsible for translating new policy legislation and guidance into team practice. A Team Leader is accountable for the consistency and quality of individual and team practice and performance in undertaking person centred strengthbased assessments and analysis of strengths needs and risks planning implementation and review appropriate to the role and function of the team and service area.
As a Team Leader you will be responsible for developing and maintaining effective partnership working with local agencies and groups in the team area especially Primary Health Community Health Elected members communities and voluntary/community groups. The will also be required to represent the voice of Adult Social Care at system partnerships (examples include system escalation calls and Primary Care Network meetings).
This is a public/customerfacing role where the statutory English language requirement for public sector workers applies.

You will be a social worker qualified with a Social Work Degree Social Work Qualification (Dip. SW) or equivalent and registered with Social Work England who will have:
