Job Description
arysville Post-Acute is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team.n this role you will assess plan and implement individualized treatment programs to help patients recover from injuries manage pain and improve their overall mobility and function. cite turn0search4 Key Responsibilities: Patient Evaluation: onduct comprehensive assessments to determine patients physical conditions and functional limitations. Treatment Planning: evelop personalized therapy plans based on evaluation findings and patient goals. Therapy Implementation: dminister therapeutic exercises manual therapy and other interventions to enhance strength flexibility balance and coordination. Progress Monitoring: egularly assess and document patient progress adjusting treatment plans as necessary to achieve optimal outcomes. Patient and Family Education: rovide guidance to patients and their families on therapeutic exercises and strategies to maintain progress post-discharge. Collaboration: ork closely with a multidisciplinary team including physicians nurses and other therapists to ensure comprehensive patient care. Qualifications: octorate or Masters degree in Physical Therapy from an accredited program. urrent and valid Physical Therapist license in the state of California. PR certification is preferred. xperience in a skilled nursing facility or rehabilitation setting is preferred. trong interpersonal communication and organizational skills. compassionate patient-centered approach to care.
