Job Title: Memory Care Nurse Supervisor

Reports To: Assisted Living Nurse Manager

Job Summary:

The Memory Care Nurse Supervisor is responsible for ensuring the general wellbeing of memory care residents through the delivery of highquality services that comply with applicable state and federal laws and regulations. Responsible for delivery of all nursing services to the residents of the Memory Care Residents in conjunction with Assisted Living Nurse Manager.

Job Qualifications:

• LPN licensed by the Commonwealth of Virginia
• UAI ISP and CDP certified or obtain within six months.
• Maintain a current license to practice as a Licensed Practical Nurse from the Board of Nursing of the Commonwealth of Virginia.
• Two years of gerontological nursing experience.
• Possess strong organizational and time management skills.
• Ability to provide nursing services to the elderly.
• Ability to effectively supervise a diverse employee work group.
• Excellent oral and written communication skills
• High degree of interpersonal relations skills and capability of effectively relating to a variety of people and personalities. Ability to maintain a positive constructive attitude among staff and between departments.

Key Duties and Responsibilities:

1. Support the Mission Values and Vision of LifeSpire of Viriginia.
2. Responsible for supervision coaching and counseling of team members.
3. Assist Nurse Manager with ensuring delivery of nursing services to all AL/MC residents according to professionally recognized nursing practices. Directly responsible for coordination of physician services within the memory care neighborhood.
4. Review 24hour report daily and follow up on pending issues with nursing and other team members.
5. Responsible for monthly medication and treatment cart review medication destruction and routine medication refills.
6. Collaborate with nurse manager on annual updates to medication management plan.
7. Monitor for effective communication of resident care:
   1. address resident and family concerns timely.
   2. ensure proper utilization of resident care plans by all appropriate nursing staff.
   3. monitor for effective communication within and between nursing shifts.
   4. maintain and protect the confidentiality of resident information at all times.
8. Develop and implement nursing systems that ensure consistent delivery of quality resident care and services:
   1. identify trends in resident care and develop plans for corrective action as needed.
   2. participate in Risk and QAPI meetings
9. Develop and maintain ISPs UAIs Fall and Risk assessments for all memory care (MC)residents.
10. Complete skin assessments weekly for MC residents to include working with MC RMA and CNA to track resident skin conditions. Provide and/or coordinate proper wound care services. Ensure proper prevention measures are in place.
11. Process physician orders.
12. Responsible for tracking and follow up of labs xrays and special procedures.
13. Oversee fall prevention and management programs.
14. Complete investigations for incident reports and subsequent interventions.
15. Perform assessments of residents upon admission leave of absence and post falls.
16. Attend monthly resident care plan meetings with families as part of the interdisciplinary team.
17. Develop and maintain professional working relationships with outside physicians pharmacy consultants dietitians and therapy partners.
18. Develop effective staffing and scheduling patterns to maintain continuity of resident care on all shifts and ensure that the total nursing needs of the MC residents are met.
19. Supports staff by covering medication cart for breaks callouts and as needed.
20. Participates in the oncall rotation for higher levels of care.
21. Ensure compliance with federal/state regulations regarding nursing services and Policies and Procedures.
22. Coordinate new resident admissions and level of care transfers as needed.
23. Support program manager to execute therapeutic recreational activities and engagement.
24. Assist Administrator in development and/or revision of nursing policies and procedures.
25. Commit to maintaining professional competence through participation in annual continuing education.
26. Follows all personnel policies and procedures outlined in the Employee Handbook and subsequent publications and notices (including policies on breaks meal periods and PAL time).
27. Maintains thorough job knowledge and understanding of:
