Experience working with multiple providers or clinics is highly preferred.

Job Summary
The Outpatient Navigator Medical Weight Loss & Bariatric Liaison plays a dual role in supporting patients on their weight loss journey while serving as the primary connector between the North Carolina Bariatric Institute (NCBI) and affiliated independent bariatric surgeons. This position ensures patients receive seamless coordinated care from medical weight loss through surgical intervention when appropriate while strengthening communication and collaboration across care teams.

Frye Regional Medical Center located in the beautiful foothills of North Carolina is a 355bed acute care hospital 81bed Behavioral Health facility and more than 70 primary and specialty care providers.
Essential Functions
Patient Navigation (Medical Weight Loss):Serve as the primary point of contact for patients enrolled in the medical weight loss program.
Guide patients through program phases from initial consults and diagnostics to ongoing weight loss support.
Coordinate schedules for physician visits lab work nutrition consults and behavioral health appointments.
Conduct regular followups to assess progress troubleshoot challenges and keep patients motivated.
Provide education on lifestyle changes medication options (e.g. GLP1s) preop requirements and postop care expectations.
Bariatric Liaison Duties:
Act as the communication bridge between NCBI and independent bariatric surgeons and their practices.
Ensure timely sharing of clinical documentation surgical referrals and pre/postop care plans.
Facilitate collaborative case reviews and multidisciplinary care coordination meetings.
Monitor patient transitions from medical weight loss to surgical evaluation and back to postoperative medical followup.
Maintain accurate records of all interactions handoffs and communications in EMR and liaison tracking systems.
Resolve issues related to scheduling records or communication gaps between NCBI and surgeon offices.
Coordinate with patients and providers for backontrack and stayontrack programs in conjunction with NCBI
Tracking outcomes and managing backontrack and stayontrack programs
Administrative & Program Support:
Track patient outcomes document progress and ensure compliance with program protocols.
Assist with patient insurance authorizations and ensure required documentation is completed.
Support program marketing community outreach and patient education initiatives.
Participate in quality improvement efforts and data collection.
