JOB SUMMARY

Under the supervision of the Social Services Supervisor the Social Services Liaison will embody the Masonic Homes mission statement by incorporating the four pillars of safety personal connection experience and efficiency into their jobs essential functions. Handles all logistical considerations for internal moves communication with families and similar projects. Assist residents with their needs and direct them to the correct department follow up visits clerical functions and special projects as assigned.

ESSENTIAL FUNCTIONS

The duties listed below are intended only as illustrations of the various types of work that may be performed. The omission of specific statements of duties does not exclude them from the position if the work is similar related or a logical assignment to this class.

  • Inputting the new resident information in S2 database.
  • Inform Environmental Services and Assisted Living Supervisors of Internal moves.
  • Assumes responsibility for establishing and maintaining professional working relations with residents staff members visitors callers and business professionals.
  • Maintains the room roster updated and sends it out monthly to various departments.
  • Assist with phone/office coverage at the front desk as needed.
  • Participate in developing and updating care or service plan calendar
  • Is responsible for resident deceased files to be placed in the archives room yearly and coordinates with the Chaplin of any missing information in the files.
  • Keeps management appropriately informed of area activities and of any significant problems.
  • Conducts residents safety room inspection on a quarterly basis in partnership with EVS.
  • Assists residents with a variety of support i.e. Voters registration forms DMV forms Para Transit applications and occasional online shopping for those residents without families.
  • Uploads necessary documents to Matrix Care to residents electronic file and makes necessary updates on emergency contacts and other pertinent information.
  • Coordinates and supports the Low Vision group and Special Friends with educational speakers and is creative in assisting these groups grow.
  • Assign mailbox to new residents.
  • Collaborates with the nursing team residents with packing/unpacking for internal moves.
  • Puts requests for pendant alarms on those residents who are moving internally.
  • Provides the laundry schedules to new residents and updates the laundry schedule as residents move to different levels of care.
  • Assist in resident room inspections for items that are reported lost/missing and completes the theft and loss report form.
  • Maintain and update AHCD and POSLT forms behind residents front door for emergencies
  • Coordinates with IT on cable setup and is responsible for the phone and TV setup for internal moves.
  • Meet with Admissions coordinator on rooms that will be for internal moves.
  • Communicate regularly with family members caregivers EVS Assisted Living team and Memory team on upcoming internal moves.
  • Maintains good communication with residents that may need additional support as they transition to the Home.
  • Performs miscellaneous clerical functions and special projects as assigned.
  • Ensures that work area is clean secure and well maintained.
  • Other duties as assigned.



Requirements

SKILLS ABILITIES AND EXPERIENCE

  • Must be able to prioritize plan and handle multiply tasks/demands simultaneously.
  • Demonstrates good judgment problem solving and decisionmaking skills.
  • Possess written and verbal skills for effective communication.
  • Ability to read write and speak English in a clear manner.
  • Ability to sit as a desk and utilize the computer telephone and other office equipment throughout the entire workday.
  • Experience working with the geriatric population preferred.
  • Works well independently and yet can function well as part of a work team.
  • Excellent knowledge and skill in working crossdepartmentally able to influence team members outside organization agree on deadlines and project steps and create clear timelines and communicate priorities and agreements to accomplish common projects such as moveins or room changes in a way that adds to personal connection to the Resident and to the team.

Any combination of education and experience that would provide the necessary knowledge and abilities listed typically:

  • High School diploma or equivalent experience.
  • Must be highly skilled with technology and have well developed office skills such as answering multi phones line systems and dispatching calls correctly and swiftly fax email and Microsoft office.
  • Ability to exercise good judgement and initiative in accomplishing responsibilities.
  • Exceptional ability to handle highly emotional. Potentially emergency situations with preparation skill and the needs of the residents and the Homes in mind.
  • Trained extensively in emergency response and recovery methods demonstrated by conduct in emergency settings.
