Job Description

The Sunnybrook Academic Family Health Team (SAFHT) is seeking an enthusiastic Occupational Therapist to join our primary care clinic. The successful candidate will have excellent communication and interpersonal skills to work collaboratively with our interprofessional team.

DUTIES & RESPONSIBILITIES:

  • As an integral member SAFHTs interprofessional health care team you will deliver Occupational Therapy services in accordance with SAFHT / SHSC policies and professional practice standards.
  • For adult patients with complex chronic medical conditions the Occupational Therapist will participate in Telemedicine Impact Plus (TIP) clinics to support the development of coordinated care plans with clients and interprofessional team members.
  • Home safety assessments/functional assessments
  • Assessment and prescription of mobility aids and seating
  • Working to full scope of practice including physical and cognitive assessments
  • Screening assessments for driving
  • Group interventions with members of the interprofessional team
  • Primary care outpatient assessments
  • Demonstrate a commitment to ongoing professional development
  • Participate in program development evaluation and ongoing quality improvement initiatives in the organization
  • In support of Sunnybrooks full affiliation with the University of Toronto participate in teaching occupational therapy students on an ongoing basis including acting as a preceptor for fieldwork placements; supervision of one or two student placements annually is expected.

QUALIFICATIONS:

  • Registration with the College of Occupational Therapists of Ontario required
  • Must be currently registered in good standing with the College of Occupational Therapists of Ontario
  • Evidence of liability insurance
  • Current Ontario Drivers License and access to an automobile
  • Minimum 3 years of experience as an Occupational Therapist with experience working in primary health care clinics
  • Ontario Ministry of Health and Long Term Care Assistive Devices Program (ADP) Authorizer status for mobility aids required
  • Experience in program development implementation monitoring and evaluation
  • Excellent communication and interpersonal skills
  • Proven flexibility and adaptability in a fastpaced work environments
  • Excellent time management organizational and prioritization skills
  • Commitment to clinical excellence and use of Best Practices
  • Demonstrated commitment to PersonCentred Care approach and principles
  • Ability to demonstrate reflective practice

Experience with Electronic Medical Records (EMRs). Previous experience with Telus

Practice Solutions (PSS) is an asset.
