Description

Currently we are seeking a highly motivated and compassionate individual to join our team as a Community Health Worker for the Help Me Grow Program. The Help Me Grow Home Visiting Program is a voluntary program that gives pregnant women and families the necessary resources and skills to raise children who are physically socially and emotionally healthy to learn. The goals of the program are to: Improve maternal and child health prevent child abuse and neglect encourage positive parenting; and promote child development and school readiness.


In this role you will be responsible for helping clients and their families to navigate and access community services other resources and adopt healthy behaviors. As a priority CHWs activity will promote maintain and improve the health of clients and their families. CHWs will provide social support and informal counseling advocate for individuals and community health needs. Community outreach regular client contact such as home visits will be required.



Essential Job Duties:

  • Conduct required weekly biweekly or monthly home visits to provide parents with learning opportunities that will assist in enhancing the growth and development of their children and support positive family interactions.
  • Complete and maintain accurate written and electronic database records in accordance with ODH and HFA requirements.
  • Plans effectively for each visit and to teach effective parenting skills and educate on child development milestones.
  • Assists families with developing and attaining goals developed on their Family Plan and making formal and informal connections to appropriate community services throughout the program.
  • Identify developmental concerns and make appropriate referrals to early Intervention. Continue ongoing and consistent communication with all Early Intervention providers to facilitate a seamless wellrounded coordination of services.
  • Performs related administrative responsibilities; maintains client information reports records and other documents; responds timely to emails and phone calls; attends various trainings and meetings; communicates with other agencies departments and vendors regarding required paperwork contract compliance and consults with other county agencies and/or supportive services as necessary.
  • Supports the completion of funder requirements and program assessments.
  • Organizes and maintains written documentation for participants employer and educational institutions in case notes to ensure they are complete current accurate and disseminate according to program and department requirements.
  • Maintains ongoing contacts (phone meetings and conferences) with other professionals involved including collateral contacts with community resources/services
  • Facilitate communication and family involvement in referrals to other agencies and maintaining records and other documentation related to the referrals.
  • Encourage and foster the familys independence in decision making about the services they are receiving.
  • Explore future planning with families including transition to Head Start or other preschool service option initiating 6 months prior to childs third birthday.
  • Participate in program evaluation activities as requested.
  • Serve as a mandatory reporter as outlined in Ohio Revised Code. Effective communication with Child Protective Services (CPS) to promote the safety and welfare of children and families.
  • Plan and organize own work to meet priorities and deadlines (daily weekly monthly quarterly yearly)
  • Confer with supervisors regarding progress concerns and recommendations.
  • Completes and updates various assessment tools and data entry into the required data system accurately and within required timeframes.
  • Utilizes curriculum and implements model as required by program.
  • Maintains the confidentiality of client or collateral interaction within Agency defined standards.
  • Participates in staff training supervisory and staff meetings.
  • Maintains regular and reliable attendance.
  • Exhibits positivity flexibility and a willingness to provide inservice training and take on new responsibilities as requested or required.
  • Be a positive role model for all health center staff.
  • Other duties as assigned.

Requirements

Qualifications:

  • High School Diploma or its equivalent.
  • 2 professional references
  • Successful completion of a BCI fingerprint screening.
  • Successful completion of a Community Health Worker formal training program such as from a college or other education institution within one year of date of hire.
  • Ability to be insured on the commercial auto insurance policy based on driving record including active drivers license and personal automobile insurance and reliable transportation.
  • Employment offers conditional upon completion of a background screen including fingerprinting and a drug screen
  • Within the first 90 days of employment: BLS and completion of health screenings as outlined in the credentialing policy are required for successful completion of orientation.
