Job Description

BERKLEY PUBLIC SCHOOLS

Position Description: School Nurse

Position Goal: To promote and maintain the good physical well being and health of pupils and staff. To aid children and youth to develop to their full potential. To practice the knowledge and understanding of the normal growth and development of children and youth and the principles of health care and guidance. To coordinate the resources of the school home and community as they pertain to the total health of the student and the staff.

Qualifications:

  1. Current license to practice as a Registered Nurse in Massachusetts.
  2. Possession of a minimum of a Baccalaureate degree in Nursing from an accredited institution.
  3. A minimum of two years child health community health or other relevant clinical nursing experience.
  4. Completion of an orientation program based on the requirements for delivery of school health services as defined by the Department of Public Health.
  5. Current certification in cardiopulmonary resuscitation and first aid from a recognized provider.
  6. Appropriate DESE Certification and passing score on MA DESE Communication & Literacy Skills Test.

Reports to: School Principal and work cooperatively with the school physician teachers and other school staff.

Performance Responsibilities:

  1. Coordinate state mandated yearly screenings Preschoolgrade 8 (vision hearing height weight postural BMI):
  1. record on school health records
  2. letters of referral
  3. followup phone calls
  4. refer parents in financial need to available service free of charge.
  1. Administer medication and monitor students who require medication in the school setting:
  1. Recording
  2. obtaining necessary physicians order and parent consent forms
  3. knowledge of potential side effects and observation of students
  4. emergency care plans for students with EPIPENS.
  1. Monitor mandated immunizations and exclude students who dont meet state and local requirements:
  1. review all Preschool8
  2. update records as new immunization are obtained
  3. letters notifying lack of and need for or update of certain immunizations
  4. followup phone calls
  5. refer parents in financial need to available resources to receive services free of charge
  6. change and develop new forms with state required changes.
  1. Assist school physician with mandated physical examinations Grade 4 & 8:
  1. notify parents
  2. letter of referrals
  3. followups
  4. chart findings on all school health records of students who have service provided by the school
  5. followup on students who choose service to be provided through own physician obtain updated report to place on record through letters/phone calls.
  1. Provide acute and emergency care in cases of injury or sudden illness for both students and staff:
  1. fill out employee accident reports and submit to town office
  2. document on student records
  3. develop forms for reporting by staff i.e. recess gym classroom.
  1. Assess and evaluate students health:
  1. identify through staff parent observation information received or screenings
  2. referrals (parents agencies physician) follow up and recommendation to parent.
  1. Provide skilled nursing care and case management for children with special health care needs (includes regular education special education):
  1. develop individual health care plans/504 plans/medication adm. plans
  2. develop emergency plans if applicable
  3. meet with teachers/staff involved with student share student needs plan of care/action and address questions and concerns they may have
  4. meet with parents and address their concerns obtain their approval of care/emergency plan(s).
  1. Ensure a safe school environment through prevention and control of disease:
  1. develop infectious disease notices
  2. send notices to parents notifying of exposure or potential that child may have a particular disease
  3. exclude students known to have or probability that may have a disease that could place other students or faculty at risk
  4. notify local and state boards of health when necessary.
  1. Counsel students parents and school personnel.
  2. Plan actions for eliminating minimizing or accepting health problems that interfere with students effective learning.
  3. Assist in the identification and education of students with special health problems that affect learning and total growth.
  4. Keep and maintain students permanent health records. Share pertinent information with teachers/staff regarding students health to the extent that the law permits. Obtain parent permission through parent signing a Release of Medical Information Form.
  5. Assess pertinent family history and home situation factors make home visits where necessary.
  6. Coordinate health services with health education coordinator and other parts of the total education team. (i.e. coordinates Health Awareness Fair provides Human Sexuality Education grades 78 AIDS Education gr. 38).
  7. Respond to frequently encountered health problems i.e. child abuse/neglect drug use and abuse eating disorders venereal diseases and adolescent pregnancy.
  8. Observe school facilities and recommend modifications necessary to maintain optimum health and safety of students/school personnel.
  9. Participate as a consultant for health education programs and participate in programs as necessary for both school and community.
  10. Assume responsibility for continuing education and professional development.
  11. Assume responsibilities for inservice programs for school personnel regarding healthrelated issues first aid EPIPen training and OSHA training.
  12. Execute other duties as assigned by the Superintendent Principal and/or Administrator of Student Services.

Terms of Employment: Salary per teacher contract

Evaluation: Annually by Principal per teacher contract
