Job Description
Position Specifics: Registered Nurse (RN) Referral Specialist HSHS Home Care
Department: Home Health Care
Schedule: FullTime 40 hours/week
OnCall: One night per week & every 68 weekends
Location: Effingham IL Service Area
SignOn Bonus: Up to $10000 for external candidates
Compensation: Based on experience
Education Qualifications
- Completion of State Approved LPN Training Program is required.
Experience Qualifications
- 1 year of acute nursing experience is preferred.
- Prior experience in home health or hospice is preferred.
Certifications Licenses and Registrations
- Basic Life Support (BLS) certification is required.
- License as a Licensed Practical Nurse (LPN) in the state of practice is required.
- Valid state drivers license current automobile insurance and reliable automobile is required.

Pay Range:
$22.05 $33.08A successful candidates actual pay rate will be based on several factors including relevant experience skills training certifications and education.
