You will be updated with latest job alerts via emailReporting to the Specialty Clinical Pharmacy Manager this position is to provide the services of a licensed pharmacistwhich includes the provision of specialized clinical and/or comprehensive medication management services to patients inthe ambulatory setting to patients who range in age from neonates infants less than one year of age children andadolescents ages 1018 adults ages 1965 and the elderly over age 65.
This position is to function at higher decision making levels regarding drug therapy proactively affecting drug use bymaking recommendations at the time prescribing decisions are being made or in some cases making independentdecisions about drug therapy in cooperation with other health team members. This position is responsible for the direct clinical supervision of patients and ensuring compliance with all legal regulatory and institutional directives. Thispharmacist will interact directly with patients prescribers nurses and care coordinators to develop an interdisciplinaryapproach to patient care. Additionally the pharmacist will be responsible for the development and implementation ofadditional Specialty Pharmacy clinical opportunities and will be expected to contribute to the financial viability of theprogram department and institution.
Interprets and evaluates providers order and prescriptions for accuracycompleteness and appropriateness. Performs medication reconciliation and recognizes therapeutic problems including drug interactionscontraindications improper dosing and precautions of therapy for a highly specialized patient population. Recognizes actual or potential problems andprovides therapeutic recommendations for their resolution.
Monitors medication therapy for response and adverse with providers to ensure safe and appropriate therapy throughthe use of a Collaborative Drug Therapy Management (CDTM) agreement. Appropriately reports all adverse drug reactions drug defects and medication errors. Conducts prospective and retrospective drug utilization reviews to ensure optimal therapy.
Counsels and trains patients regarding the appropriate use of theirmedications (i.e. purpose actions dosage side effects storage precautionsetc.).
Participates in drug product selection counsels providers regarding theselection of nonformulary drugs while offering therapeutic cost effectivealternatives.
Maintains appropriate records as required by law and/or policy. Assists inthe development and implementation of the specialty pharmacy servicesprogram.
Researches and evaluates professional literature in response to questions and requests and provides drug information to clinic staff.
Assists with devising developing and operationalizing programs to ensureproper use of medications and continuous quality improvement.
Participates in professional developmental programs to ensure progressivepharmacy practice and actively demonstrates service excellence.

The pharmacist will experience competing demands on their time from clinic staff and/or patients and will
need to effectively multitask and prioritize demands. The pharmacist must deal with intense situations on a
daily basis. The expectation will be to provide clients with excellent customer service. Additionally within
the healthcare setting there can be a significant lack of control over the work pace with frequent interruptions
(work is often dictated by external factors) that may lead to mental fatigue or stress.
