Responsibilities

  1. Assist in preadmission and admission process with customers as needed as backup to Admissions Department personnel. Duties to include providing tourssigning of paperwork etc.
  2. Interview clients and families to determine financial status and need for Medicaidassistance.
  3. Assist families in applying for clients Medicaid (ICP) approval. Attend annualreview for Medicaid eligibility if families unable to attend.
  4. Assist facility bookkeeper in tracking and reporting status of Medicaid Applications/Approvals.
  5. Assist Social Services Director as needed with interviewing residents familiesto obtain relevant past and present social histories including past behaviors adjustments and lifestyles.
  6. Documentsprogress and updates plans of care at interdisciplinary conference asneeded.
  7. Assist residents and families with adjustment to admission to facility by providing oneonone counseling and/or group discussions.
  8. HelpSocial Services Directory with planning of post discharge services andfollowup care.
  9. Maintaineffective communication with residents families health careprofessionals and facility staff.
  10. Assistresidents and families in any situation as needed.
  11. This person may have access to protected health information in the patientsfinancial and medical records to conduct job duties.

Qualifications

  1. Minimumof High School diploma or equivalent. Bachelors Degree in Social Work or Human Services is preferred.
  2. One yearof supervised social work experience in a health care setting working directly with individuals.
  3. Experience or knowledge of Institutional Medicaid Program (ICP) qualifications andapplication process preferred.
  4. Ability to work with a variety of people in situations varying from community andhealth care settings.
  5. Able to make decisions concerning procedures based on evaluations and interpretations of clients case histories
