The Surgery department has an exciting opportunity for a Nurse Navigator 1 (H) position. The incumbent is the initial point of contact for a patient entering the health system and assures timely scheduling of the first appointment and coordination of care after completion of the first appointment and supports the patient throughout the care continuum. The Nurse Navigator 1 (H) serves as a liaison between patients families caregivers and the multidisciplinary care team. The incumbent demonstrates a commitment to quality patient care implements creative and innovative ways to meet the diverse needs of the patients and ensures best practices.

DEPARTMENT ADDENDUM

Department Specific Functions

Supports clinical functions within scope of practice for all Divisions within the Department of Surgery. Assist with all inperson virtual and/or telephone patientrelated clinical questions and concerns. Communicates effectively with Physicians and midlevel providers to effectively address and resolve medical concerns. Serves as a clinical resource to the surgical scheduling patient access and other nonclinical teams within the Department of Surgery. Tasked with building relationships with clinical and nonclinical staff in order to streamline intra and interdepartmental processes. Serves as a point of contact for coordination of care across multiple departments agencies and/or organizations. Responsible for completion of medical related forms such as FMLAs and DWC25 forms. Discuss clinical information with insurance companies to obtain necessary approvals for surgery/treatment. Contact outside providers/hospitals to secure medical records. Documents all patient related encounters appropriately in our electronic medical records. Functions as a professional role model for behavior and makes positive impact on peer morale. Support other departmental functions as needed.
