Description

The Employment Coach will assist the Workforce Development program with helping persons supported prepare for and maintain employment that enhances community inclusion and will help people supported achieve their identified employment outcomes/goals. The Employment Coach will assist with fading supports and/or identifying natural supports and/or enabling technology to increase independence. Performs all duties necessary to meet Stateapproved objectives and requirements. Maintains a highquality personcentered program in accordance with Company standards mission and vision. Helps people prepare for employment.

ESSENTIAL FUNCTIONS

  • Familiar with each person supported within program area utilizing information found in the Individual Support Plan (ISP) Individual Plan for Employment (IPE) Person Centered Support Plan (PCSP) as well as the individuals own vision for a meaningful life. Utilize ISP PCSP and IPE to assess individual needs and interest in the community and to develop goals and objectives.
  • Provide intensive job coaching instruction and training for job placements and/or worksites. Consistently model appropriate work maturity skill appropriate appearance and dress suited for representing Emory Valley Center to community employers.
  • Provide oneonone training tailored to the needs of the person supported. Ensure safety is always being maintained in the work environment.
  • Provide service for persons supported regardless of location by creating and maintaining an atmosphere that is welcoming understanding communicative and supportive of person supported. Maintain and model positive professional working relationships with persons supported.
  • Willing and able to work with persons supported with incontinence and/or personal care in a respectful and professional manner when necessary.
  • Participate in meetings as necessary to report on the persons supported progress.
  • Accurately completes information related to ISP/PCSP and IPE activities and all persons supported documentation: maintains detailed communication notes which are completed daily after each shift monitoring records outcome summaries etc. completing necessary billing documents and reports utilizing an electronic health record and submitting forms electronically.
  • Perform job analysis to identify job duties and develop a plan as to how to train the person supported to work in a selfsufficient manner utilizing natural supports and/or enabling technology as needed for independence on the job with the plan for fading the employment coach from the first day on the job.
  • Follows all Therapy Plans (OT PT SLP and Nutrition) and Behavior Support Plans as defined in the Information and Training Specific to the Person (ITSP) ISP PCSP or competency based trainings.
  • Maintain regular attendance in accordance with personnel manual and continuity for those people supported.
  • Other duties as assigned by Employment Specialist Workforce Development Director Vice President of Operations and President.

KNOWLEDGE AND SKILLS

Ability to exercise good judgment and use common sense.

Knowledge and understanding of local and State regulatory Supported Employment Services.

Knowledge and understanding of Company and DIDD operation policies.

Understanding and commitment to community participation for individuals with disabilities and Company values.

Flexibility to deal professionally with people at a variety of levels both internally and externally.

Ability to present facts and recommendations effectively in oral and written form.

Excellent organizational interpersonal and communication skills that promote professional teamwork.

Excellent computer skills.

Ability to lift 50 pounds (weight of average manual wheelchair when chair is empty).

EDUCATIONAL AND WORK EXPERIENCE

Must be at least 18 years of age.

High school diploma (or equivalent) with 6 months of experience in field of intellectual and developmental disabilities or closely related field.

REQUIRED LICENSES AND/OR CERTIFICATES

Department of Intellectual and Developmental Disabilities (DIDD) designated online training Managed Care (MCO) designated online training Vocational Rehabilitation (VR) designated training

First Aid CPR and Medication Certification course within first 90 days of employment and any other required training.

Emory Valley Center Orientation.

Valid Tennessee drivers license with F endorsement and an acceptable driving record.

Department of Transportation physical exam as applicable.

Person Centered Training.

Obtain and maintain Training Resource Network Certification (TRN)

WORKING CONDITIONS

Will provide services after hours on weekends and holidays as scheduled and according to the Companys Policy and person supported s work schedule.

Frequent lifting stretching and other physical exertion during positioning of individuals supported or equipment.

May work with people who exhibit aggressive or violent conduct.

While performing the duties of the job employee travels by automobile will utilize own vehicle and is exposed to changing weather conditions.

May occasionally assist with wheelchair transfer of nonambulatory individuals.

May be exposed to various medical conditions and communicable diseases.

Work will take place in the community and in the homes.
