Pinnacle Home Care Floridas largest Medicarecertified home health provider has been delivering highquality patientcentered care for over two decades and were looking for IVCertified Licensed Practical Nurses (LPN) to join our awardwinning team.
Key Responsibilities
- Administer skilled nursing care under RN supervision.
- Perform curative rehabilitative or preventive care tasks as outlined in patient care plans.
- Utilize clinical expertise and nursing education to make informed independent decisions that support patient care and promote positive outcomes.
- Complete visit reports promptly and accurately within the electronic medical record system to support highquality care and regulatory compliance.
- Confirm daily visit schedules to ensure seamless coordination with other staff.
- Demonstrate awareness of the agencys Quality Assurance and Performance Improvement (QAPI) program and participate as required.
- Understand and adhere to the agencys process for managing and addressing customer complaints.
- Maintain acute awareness of patient rights ensuring these rights are upheld in accordance with the agencys service agreements and admission policies.
- Understand the agencys policies regarding appropriate patient discharge and reasons for discharge.
Qualifications
- Must have a current LPN or LVN license (statespecific).
- IV Certification
- Graduate of an accredited nursing program.
- Valid BLS certification.
- Minimum one year of nursing experience (home health or medical/surgical preferred).
- Excellent communication organizational and time management skills.
- Must have a valid drivers license and access to a reliable and insured vehicle.
- Ability to visit homes that may not be wheelchair accessible.
- Sufficient hearing eyesight and dexterity to perform physical assessments and demonstrate care techniques.
- Engage in physical activities such as walking standing stooping and occasional lifting or turning of patients.
