Your Impact as a Home Health Physical Therapist:
- Provide oneonone care in clients homes ensuring they receive topquality therapy.
- Perform mobility and functional assessments develop individualized care plans and help clients regain strength and independence.
- Educate clients and their families on exercises safety measures and strategies to enhance daily living.

What Were Looking For:
- Active NC Physical Therapist (PT) license
- Graduation from an accredited PT program (APTA or CAHEA/AMAapproved)
- Strong time management communication and problemsolving skills
- Ability to work independently while staying connected to a supportive team
