Job Description
We are working closely alongside a Local Authority in Greenwich to assist with the appointment of a Personal Advisor Care Leavers on a 3month contract likely to be extended at clients discretion. Please apply with your CV for immediate consideration.
Hours per week: 17.50
Rate of Pay: 18.30 23.05 per Hour
Responsibilities:
- Assess the needs of young people who are eligible for leaving care services by actively contributing to the assessment and preparation of the Pathway Plan so that young people are supported to achieve independent living.
- Contribute to the involvement and participation of young people in the development and delivery of their Pathway Plans and develop creative approaches that will fully engage young people in the process.
- Ensure that the Pathway Plan is reviewed at the prescribed intervals and be responsible for convening additional reviews when circumstances significantly change.
- Revise and amend assessments to inform the Pathway Plan.
- Coordinate how services are provided for the young person in a timely way developing constructive professional relationships to enable the young person to succeed in their transition to independence and adulthood.
Experience and Knowledge:
- Applied knowledge of safeguarding and youth justice legislation and principles underpinning good practice
- Experience of working with young people.
- Working Knowledge Of Effective Youth Justice Practices
Please note:
- You should be available to work immediately or at a short notice.
- You should have right to work in U.K
- This role requires an Enhanced DBS
