About the team
The Heart and Lung Program is responsible for a range of comprehensive treatments for severe heart and lung diseases. The Program is structured under three Specialty Medical units Cardiology Cardiothoracic and Respiratory Medicine. Our Heart & Lung Transplant service provides a referral assessment and transplantation service for adults with end stage heart and/or lung disease as well as the National Paediatric Lung Transplant Service. A multidisciplinary team that includes surgeons physicians cardiologists allied health and nursing staff manages each patient journey.
Position Summary
The role uses expert clinical knowledge to provide advanced patient triage and assessment that informs and contributes to the development of optimal care planning and treatment care pathways. The CNC uses this specialist knowledge to educate clinicians patients families and carers which in turn assists their understanding of their diagnosis and subsequent treatments.
The Heart & Lung Transplant Coordinator After Hours Clinical Nurse Consultant is the primary contact in the management and coordination of all heart & lung donor referrals and transplant coordination for all heart and lung recipients for the Alfred after hours.
