Hospice / Registered Nurse / Case Manager
We have recently engaged with a very progressive well run and financially stable Hospice organization in the Roswell County area of Georgia. They are a familyowned organization that is committed to patient satisfaction clinical excellence and quality care for each member of the family.

Responsibilities:
- The RN Case Manager Will Maintain a Case Load of About 1718 Patients
- Seeing Patients in Facilities Mainly
- The RN Case Manager Will be Responsible for Backup Call
Requirements:
- Registered Nurse in the State of Georgia
- 1 year of Hospice or Palliative Care ExperiencePreferred
- 2 years of inHome Patient Care ExperienceRequired
- Familiarity with EMR System KanTimePreferred
