Job Description
CCM Care Coordinator (LPN)
Full Time (MondayFriday)
No Weekends and No Holidays
Keystone Health is seeking a fulltime CCM Care Coordinator (LPN) to join the team.
This position is responsible for partnering with the RN Care Managers and providers to manage a defined cohort of patients within theChronic Care Management (CCM)program. CCM Care Coordinators assist with establishing patient goals and providing virtual clinical support to improve patient outcomes and quality of life. Care Coordinators serve as a patient pointofcontact and as an information resource for the patient the care team and the family. Some of the responsibilities include provision of individualized patient education and care coordination medication reconciliation identifying and addressing gaps in care assessing social drivers of health and facilitating connections to appropriate community resources.
Qualifications:Current PA LPN license minimum of 2 years working in a clinical setting. Current CPR certification is required. The applicant needs to have strong communication and critical thinking skills be well organized and able to work both independently and in a team setting. Strong skills with Microsoft Office and Excel are a plus. Bilingual skills preferred but not required.
