We are currently seeking a Minimum Data Set (MDS) Coordinator to join our team following a valued team members retirement. This is a highly collaborative role within a supportive patientfocused team where your attention to detail regulatory knowledge and clinical insight will be highly valued. As a MDS you will be responsible for the following job duties and responsibilities:

  • Develop implement and evaluate resident assessment care plans with collaboration of other health professionals to optimize patient wellbeing.
  • Assessing and monitoring patient health to meet CMS MDS/RAI state and federal compliance requirements.
  • Oversee the completion of an MDS that is comprehensive accurate standardized and reproducible.
  • Coordinates the completion of Discharge Tracking Forms and Return Tracking forms.
  • Reviews and distributes Quality Indicator data and other reports to improve MDS accuracy and compliance.
  • Attends IDT meetings per facility schedule.
  • Provide educational training related to Medicare MDS/RAI process to staff as needed.
  • Performs other duties as assigned.

FTE: 0.9; 36 hours per week

Work Schedule: MondayFriday; Day shifts

Work Environment: Hybrid

Required Qualifications:

  • Current Minnesota RN or LPN License
  • Maintains current CPR certification
  • Previous MDS/RAI work experience preferred

Union Position: No

Benefits:

Benefits are available for nonunion staff working 24 hours (0.6 FTE) or union staff working 30 hours (0.75 FTE) per week and are effective the first of the month after 30 days of employment.
