Job Description

TITLE:Special Education Transition Case Manager

POSTION TYPE:SPEC

HIRING SPECIFICATIONS:


Required:

  1. Bachelors degree in human services or related field.
  2. Successful work experience in human/social programs which provide services/benefits to students and families with special needs.
  3. Experience in planning organizing implementing and monitoring transition servicesfor students/families.

Desired:

  1. Masters degree in human services or related field

Skills Knowledge and Abilities:

  1. Effective communication verbal and written communication skills including patience and understanding in communicating with persons being served.
  2. Knowledge of community resources.
  3. Ability to advocate for families in accessing needed resources.
  4. Ability to function as a member of a multidisciplinary team working cooperatively with both school and agency staff.
  5. Sensitive to the needs and barriers experienced by lowincome and innercity populations.
  6. Experience in assisting individuals identify and prioritize strengths/needs as well as developing plans for reaching their goals.
  7. Ability to provide own transportation and the appropriate transportation of clients when needed.

BASIC FUNCTION:

Provide leadership to staff and building administration in the area of Transition Services for students with IEPs. Foster the development of community resources regarding work based learning and other transition supports and services in the school and community. Manage the programming and resources for the 4 program within a special school setting to ensure a safe efficient and effrective learning environment for students.

ESSENTIAL FUNCTIONS:

1. Facilitate ongoing professional development for staff surrounding transition and available community resources.

2. Connect students and families with community resources that provide services and entitlements available to meet their needs.

3. Advocate for families in accessing needed resources.

4. Meet regularly with families in their homes/other locations that are convenient for the family.

5. Conduct home visits to gather information on needs for resources encompassing the whole child.

6. Collaborate with community agencies to ensure that students have connections needed to be successful outside of the school environment.

7. Collaborate with district transition services staff members to develop action plans focused on assisting students with IEPs gain access to work based learning opportunities.

8. Design specialized programs and curriculum for family and group education.

9. Monitor the services provided to students and their families regarding transition services.

10. Develop and implement summer programming and services to students with disabilities and their families.

11. Visit home schools to assess needs to ensure that transitions are smooth. Meet with teachers administrators parents/guardians and students.

12. Act as the building contact for teachers administrator and advocate.

13. Function as member of interdisciplinary school team.

14. Provide prompt ongoing followup to school and agency staff regarding client status.

15. Regularly attend child student team and other similar meetings with assigned building for the purpose of communication problemsolving and service coordination.

16. Participate in building faculty meetings parent/teacher conferences inservice activities and service on school committees when appropriate.

17. Assess the comprehensive needs of children/youth and their families as well as their current and potential strengths and weaknesses.

18. Maintain accurate records and complete required reports.

19. Work effectively with public and private agencies school personnel and community persons.

ADDITIONAL ESSENTIAL FUNCTIONS (for every DMPS position):

  1. Attend work on a prompt and regular basis.
  2. Maintain confidentiality.
  3. Maintain satisfactory and harmonious working relationships with the public students and other employees.
  4. Assist with other duties as may be assigned

FT/PT:Fulltime

DAYS: 225 HOURS: 8 hour day

PHYSICAL REQUIREMENTS:

Frequent standing and walking.

Occasional sitting bendingstooping climbing stairs driving lifting (50lbs) carrying (10 feet).

WORKING CONDITIONS:

Mostly inside occaisonal outside extremes of temperature/humidity
