What Youll Do:
- Conduct client assessments and create personalized care plans.
- Coordinate medical home care and community services.
- Advocate for clients with healthcare providers and families.
- Provide crisis intervention and family caregiver support.
- Maintain HIPAAcompliant records using EHR software.
- Bachelors or Masters in nursing social work gerontology or related field.
- 2 years in care management or geriatric care.
- ALCA membership or certifications (CMC CCM) preferred (or willingness to obtain).
- Strong communication organization and problemsolving skills.
- Valid Illinois drivers license and ability to pass background checks.
- Competitive pay ($60$80/hour) and flexible schedules.

- Education: Bachelors degree (Masters preferred).
- Experience: 2 years in care management or related field.
