Description

We are hiring: JailBased Care Coordinator

Starting wage: $22.00$25.00/hour plus experience considered.

Job type: Fulltime Nonexempt

Schedule: MondayFriday 8am5pm.

What will you do in this position

As aJailBased Care Coordinator you will play a vital role in supporting individuals transitioning from incarceration back into the community through theJailBased Program. Your work will focus on care coordination and collaboration with a multidisciplinary team of community organizations. This is a highly visible forwardfacing position that serves as a key liaison with County leadership.

Key Responsibilities:

  • Provide direct supportto clients referred to the program helping them navigate reentry into the community.
  • Conduct regular inperson visitsto local jailto engage with clients build rapport and assess needs.
  • Facilitate weekly staffing with program partners.
  • Conduct intakes for all new program clients.
  • Collaborate with community partners referral sourcesand agencies to ensure a seamless continuum of care.
  • Assist clients in identifying and accessing essential services including housing employment healthcare and other resources.
  • Make appropriate referralsand follow up to ensure service connection and continuity.
  • Maintain accurate and timely documentationof client interactions case notes and progress.
  • Participate in team meetingsand interagency collaborations to support client outcomes and program goals.

What Skills do you need

The JailBased Care Coordinator role requires the following:

  • ABachelors degreewith at leastone year of experiencein social services mental health substance use treatment or corrections.
    • AnAssociate degreewithtwo years of relevant experience will be considered.

  • Excellentinterpersonal and communication skills.
  • Proficiency withcomputersandtyping as the role involves consistent documentation of caseloads.
  • Proven ability tobuild and maintain effective working relationshipswith external agencies and partners.

Additional Requirements

  • Must successfully pass afederal drug screeningandbackground check.
  • Avalid drivers licenseis required.

PTO

  • Accrual: Start accruing PTO from your first paycheck. Fulltime (40hour) employees with 02 years of employment accrue 4.65 hours per pay period totaling 15 days a year.
  • Increased Accrual with Tenure: Accrual rates increase at 3 years 5 years and every subsequent 5year mark up to 25 years.
  • Pro Rata Basis: Accrual rates for employees working 2030 hours per week are on a pro rata basis.

Paid Holidays

  • FullTime Employees: Enjoy 11 paid holidays.
  • PartTime Employees: Employees working 2030 hours per week also receive paid holidays on a pro rata basis.
