We are seeking a skilled and empathetic individual to join our team as aCommunity Support Team (CST) Qualified Professional (QP).This role is crucial to the delivery of highquality mental health and substance use services helping clients achieve their recovery goals and improve their overall wellbeing. The ideal candidate will have strong clinical expertise a passion for communitybased work and a commitment to making a positive impact in the lives of those we serve.
What you will do
As a CST Qualified Professional you will be responsible for providing direct care and coordinating services for individuals with mental health and substance use challenges. Working under the supervision of the CST Team Lead you will play a key role in assessing client needs developing treatment plans and ensuring that all services are delivered effectively and in accordance with best practices.
Your responsibilities will include:
Conducting comprehensive assessments of clients to identify strengths needs and goals.
Developing implementing and monitoring individualized treatment plans in collaboration with the client family and CST team members.
Providing direct therapeutic interventions including individual family and group therapy as well as crisis intervention when necessary.
Coordinating and linking clients to community resources including housing employment education and healthcare services.
Advocating for clients within various systems such as healthcare judicial and social services to ensure they receive the support they need.
Monitoring client progress and adjusting treatment plans as needed to ensure positive outcomes.
Maintaining accurate and timely documentation of all services provided in accordance with agency policies and regulatory requirements.
Collaborating with other CST team members including the Team Lead Peer Support Specialists and other professionals to ensure a holistic and integrated approach to care.
Participating in regular team meetings and case conferences to discuss client progress and coordinate care.
Promoting the values of recovery personcentered care and cultural competence in all interactions with clients and team members.

A Bachelors degree
A minimum of 2 years of experience working with individuals with mental health or substance use challenges preferably in a communitybased setting.
Strong knowledge of evidencebased practices in mental health and substance use treatment.
Excellent communication organizational and problemsolving skills.
A commitment to personcentered care and cultural competence.
A valid drivers license and reliable transportation.
