DescriptionMayo Clinics Department of Pharmacy and Division of Pulmonary and Critical Care Medicine are seeking a dedicated and skilled Pulmonary Ambulatory Care Pharmacist to join our Medication Management Services (MMS) team. This fulltime position will be based out of Rochester MN with flexible working hours that may vary between 7 a.m. 4 p.m. and 8 a.m. 5 p.m. Monday through Friday.
In this role the pharmacist will build a practice within the Pulmonary Department expanding service lines in vasculitis interstitial lung disease and Mycobacterium and Bronchiectasis Clinic. The primary responsibilities include providing direct patient care (both inclinic and virtual visits) and indirect patient care (supporting medication therapy processes assisting with patient provider and care team inquiries). This position also offers opportunities for administrative tasks and process improvement activities related to patient care as well as practice development and expansion within the department.
In addition to clinical responsibilities the pharmacist will actively contribute to quality improvement initiatives research efforts and education programs. You will work closely with a diverse team including physicians advanced practice providers nurses and other clinical staff to ensure optimal pharmacotherapy for patients.
Key Responsibilities:
- Document and complete billing processes for pharmacist activities.
- Collaborate in protocoldriven practices and/or collaborative practice agreements as applicable.
- Contribute to quality improvement and resource management projects.
- Support research initiatives within the department and clinics.
- Participate in precepting activities for students and residents.
- Contribute to continuous improvement and patient safety efforts.
- Assist with other departmental projects and initiatives.
- Collaborate with Mayo Clinic Specialty Pharmacy Infusion Therapy Center and insurance prior authorization teams.

QualificationsMinimum Qualifications
- Graduate of an ACPEaccredited Doctor of Pharmacy (Pharm.D.) program OR Bachelor of Science (B.S.) AND a minimum of 2 years relevant experience.
Additional Qualifications
- Medication Therapy Management (MTM): If limited experience in Medication Management Ambulatory Care completion of an accredited MTM certificate program is required within 12 months of hire.
- Minnesota Health Care Programs (MHCP): Must be registered or eligible to be a registered MMS/MTM provider through Minnesota Health Care Programs (MHCP).
Preferred Qualifications
- Educational Requirements: Must have a Doctor of Pharmacy (PharmD) degree OR a Bachelor of Science in Pharmacy (with at least 10 years of relevant experience if holding a Bachelors degree).
- Ambulatory Care Experience: Experience in Ambulatory Care practice especially with collaborative practice agreements (CPAs).
- PGY2 Pharmacy Residency: Completion of an accredited PGY2 pharmacy residency in ambulatory care.
- Pulmonary Disorder Care Experience: Experience providing care to patients with pulmonary disordersspecifically vasculitis interstitial lung disease and mycobacterium.
- Board Certification (BCACP): Board certification in Ambulatory Care Pharmacy (BCACP) is preferred.
- Skills and Experience in Practice and Practice Development: Prior involvement in practice initiation development belonging community service research quality improvement and leadership roles.

License and Certification
- Minnesota Pharmacist License: Must be licensed to practice pharmacy in Minnesota. or eligible to obtain within a time frame compliant with the sites applicable employee licensure policy. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
- Completion of an accredited MTM certificate program is required within 12 months of hire.
- Board Certification (BCACP): Board certification in Ambulatory Care Pharmacy (BCACP) is preferred.
