Job Description

Hospice RN Case Manager: Enjoyindependence and flexibilityin the field to visit patients one on one.

Hours:Fulltime Days Monday Friday with OnCall rotation.

Territory: The RN will have a general territory within our service area where they will provide inhome hospice care to the patients. The territory for this position would primarily be in the Southern section of the Twin Cities Metro area.

A few musthaves for this position:

  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of MN. License is active and in good standing.
  • Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or if no experience within the last five (5) years evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
    • If a new graduate 1200 verified hours of employment as a graduate or registered nurse within the last twelve (12) months or evidence of completion of a companyapproved Preceptor Program.
  • One (1) year of hospice care experience preferred.
  • Able to travel within a geographic area serviced by the office. Reliable transportation valid drivers license and proof of auto insurance is required.
  • CPR Certification is required
