Job Description
Responsibilities
Job Summary:
- Functions under the supervision of a Registered Nurse.
- Patient care is provided in the home collaboratively with the RN.
- Care provided displays respect for the patients rights dignity values confidentiality and expressed needs.
- Adaptable using evidenced based practice and information technologies while maintaining focus on providing comfort relieving pain and alleviating anxiety.
- Tasks performed are in compliance with department and hospital policies and procedures and within educational preparation as well as the Florida Nurse Practice Act.
Reports: Administrator/Home Health Care Clinical Supervisor Clinical Manager and RN(JC:01 03)

Qualifications
Required Education:
- Graduate from an accredited School of Practical Nursing
Required Experience:
- 1 year MedSurg experience is required.
Preferred Experience:
- 1 years of Home Health experience.
Required Certification/License/Registry:
- Current LPN License in the State of Florida. Must possess a State of Florida Class E Drivers License.
- Behavioral Criteria: Qualified candidates must provide a seven (7) year driving record from the Department of Motor Vehicles with no more than six (6) points and no indication of poor driving habits. The qualified candidate must be 21 years of age.
- Note: Use of personal vehicles is required. It is the responsibility of the vehicle owner to maintain adequate insurance coverage for their personal vehicle used in conjunction with their duties as an employee. TMH will reimburse the business use of a personal vehicle in accordance with procedures defined by the Internal Revenue Service.
