If so please joining Granite VNA as a Home Health and Hospice LNA where you can make a difference in the lives of our patients and their families by providing optimal care in the beautiful Wolfeboro NH area.
Reporting to the Paraprofessional Administrative Manager the Granite VNA Home Health & Hospice Aide performs personal care and designated healthrelated services for the care and wellbeing of assigned clients under the supervision of a Registered Nurse. The Home Health & Hospice Aide interacts with clients family members facility staff and other healthcare team members while maintaining professional standards.

Work Environment
The employee will be exposed to a variety of uncontrolled work environments. The employee may be exposed to all kinds of weather conditions while driving to and from work assignments. The employee may also be exposed to blood and bodily fluids virus disease and infection from patients and environmental hazards in the work environment. The employee may be required to work extended periods of time using a mobile device.
This is a Field Work position. Primary work location is in the community providing direct patient care. Not eligible for remote or work from home.

Expected Hours of Work
The employee will be scheduled to work based on the employees availability for work as well as program and Agency needs. Hours of work are defined in the Employment Agreement. Work assignments may include holiday weekend and/or oncall rotation.
