JOB TYPE: Full Time
We are seeking a compassionate organized and proactive Resident Services Coordinatorto join our team. This role is critical in ensuring a smooth transition for new residents providing ongoing support and coordinating services between departments and external providers.
BENEFITS:
Coordinate and manage all aspects of the movein process for new residents
Serve as the primary point of contact for new resident questions and scheduling
Conduct orientation on movein day and followup visits thereafter
Facilitate introductions between new residents and the community
Coordinate with other departments to ensure services and care are aligned
Complete new resident paperwork and ensure documentation is accurate and complete
Assist in connecting residents with thirdparty services as needed
Assist in touring prospective residents and their families
Ensure all required preadmission documents including the 1823 Form are received and complete before movein
Support communication and rapportbuilding with prospective families
Collaborate with the wellness department to obtain necessary physician orders
Coordinate ongoing supplemental services through thirdparty providers
Visit residents temporarily placed in hospitals rehab or higher care settings and obtain clinical updates before their return
Advocate for resident needs and ensure continuity of care
Coordinate with maintenance and housekeeping to ensure unit readiness before movein
Follow up on unit concerns or readiness issues promptly and professionally
Preferred Experience:
1 year experience in Assisted Living LongTerm Care or related senior care environment
Preferred Education/Training:
Licensed Clinical Social Worker (L.C.S.W.) preferred but not required
Required Skills:
Strong interpersonal organizational and communication skills
Ability to multitask prioritize and remain calm under pressure
Compassionate residentcentered approach to care
Professional demeanor and collaborative spirit
Must be able to understand speak read and write English
Must be able to work at a fast pace independently and as a team member with the ability to complete all job tasks within the allotted time
Must be a supportive team member contribute to and be an example of teamwork and the team concept.
Be efficient in all required skills
Must have patience tact a cheerful disposition enthusiasm and the willingness to handle difficult residents and employees.
Computer Skills required
Effective interpersonal and communication skills both written and verbal
Must be able to pass a Level 2 background check
Must be reliable show up to work at scheduled shift on time
