This Social Worker 4 position is for a vacancy located in our Home and Community Based Services (HCBS) Permanent Support Housing (PSH) section housed in Hammond Louisiana. This Social Worker position will provide services in the community and/or homes of families. Traveling throughout Tangipahoa St. Helena St. Tammany Livingston and Washington Parishes will be required.

Possession of a current Louisiana license to practice as a Licensed Clinical Social Worker (LCSW) plus:
A masters degree in social work followed by three years of social services experience.
FUNCTION OF WORK:
To serve as a licensed Clinical Social Worker and to provide independent assessment implementation as case manager of treatment plans and independent counseling with a minimum of supervision with a concentration on clients or offenders with specialized/atypical cases.LEVEL OF WORK:

Broad review from a higherlevel social worker facility manager or administrator.SUPERVISION EXERCISED:
May provide functional supervision over lowerlevel social workers and professionals in other disciplines.JOB DISTINCTIONS:
Differs from the Social Worker 3 by the presence of licensure as a Clinical Social Worker (LCSW).
Differs from the Social Worker 5A by the absence of responsibility of serving as sole social worker of a facility having full managerial responsibility over a facilitys social services program.
Differs from the Social Worker 5B by the absence of direct supervisory responsibilities.
This Social Worker provides clinical behavioral health assessments social services counselor components and supportive services to the Home and Community Based Services (HCBS) Program. The Home and Community Based Services program provides services to tenants in order to stabilize housing within the Florida Parishes Human Services Authority (FPHSA) service area. This area includes 5 parishes: Livingston St. Helena St. Tammany Tangipahoa and Washington. The position manages a specialized caseload and provides direct services for those with a substantial longterm disability expected to be of longcontinued or indefinite durations which substantially impedes their ability to live independently without supports and is of such nature that such ability could be improved by more suitable housing conditions. This includes individuals diagnosed with but not limited to: serious mental illnesses substance use disorder developmental disabilities physical cognitive or sensory disabilities or disabling chronic health conditions that qualifies them for Medicaidfunded or other funded supports and services operated by the LDH program offices for Behavioral Health Developmental Disabilities Public Health or Aging and Adult Services. These services include Community Psychiatric Support and Treatment Crisis Intervention Community Development Block Grant Only Supports Home and Community Based Support Services Psychosocial Rehabilitation and Ryan White Services. Most services are provided in community locations where the person lives works attends school and /or socializes. Extensive daily travel is required as services are provided in the community where the personserved lives works attends school and/or socializes. Due to the variety of demographical differences in persons served by HCBS clinical knowledge of children adolescents adults families mental health substance use disabilities and community is required.
