Were seeking a compassionate and collaborative Registered Nurse to join our clients PACE (Program of AllInclusive Care for the Elderly) team. This is a unique opportunity to work with a passionate interdisciplinary group delivering highquality integrated care to frail older adults across multiple care settingsincluding the clinic adult day center home visits virtual care and skilled nursing facilities (SNFs).

What Youll Do:
- Collaborate with the Interdisciplinary Team (IDT) to develop and execute individualized care plans
- Conduct comprehensive facetoface nursing assessments (physical psychosocial and behavioral)
- Deliver direct nursing care in line with physician ordersadministering medications therapies and clinical interventions
- Act as a triage nurse in outpatient settings initiating care within your scope and escalating appropriately
- Provide case management support during transitions of care (hospital SNF home)
- Educate participants caregivers and team members on care plans and best practices
- Rotate in taking afterhours calls (triaged through nurse team)
What Were Looking For:
- Active California RN License in good standing
- Passion for caring for the elderly and a holistic teambased approach to healthcare
- Experience in geriatric care home health SNF or PACE a strong plus
- Excellent communication documentation and case management skills
- Adaptability to work across various care settings
- Strong commitment to empathy integrity and continuous learning
