Job Description
Inhouse therapy team smaller daily caseloads with emphasis on optimizing time with patients! Chandler Therapy and Living Center provides short and longterm nursing and rehabilitation services to the senior population in our community. Physical Therapist Job Responsibilities: Develop and implement the Physical Therapy plan of care for facility residents and shortterm patients. Provide supervision as needed for staff Physical Therapy Assistants in delivery of the PT plans of care. Assist as needed with patient screenings for the need for Physical Therapy. Communicate progress and any barriers toward PT goals with interdisciplinary team members. Complete timely PT documentation in compliance with regulatory licensing and payor requirements. Assist with maintaining both the rehab gym equipment and treatment area in a safe manner using good infection prevention methods. Provide and document patient family and caregiver training regarding each PT patient s specific plan of care and recommendations for follow up. Education Experience and Licensing Requirements:Current Oklahoma licensure as a Physical Therapist required. New grads welcome to apply!
