Dove Healthcare West Eau Claire is seeking a new nurse case manager to add to our team.
If you are a registered nurse (RN) who is looking to take the next step in your career journey and use your experience to mentor train and educate a team of caregivers while coordinating and overseeing the care of your patients the role of nurse case manager could be your next calling!
RN Nurse Case Manager Responsibilities:
- provide compassionate care that addresses physical emotional and spiritual needs of patients and their families
- supervise the day to day activities of assigned residents and employees
- coordinate plan implement and evaluate the nursing services provided to each resident using clinical competence
- review and audit compliance with the patients plan of care and changing needs
- ensure appropriate staffing patterns are in place
- evaluate employee duties and needs
- hold employees accountable for following policies and procedures
- provide a safe comfortable and homelike environment
- oncall rotation
- other duties as assigned
Required Qualifications:
- current licensure in the State of Wisconsin as a registered nurse
- degree from an accredited school of nursing
- previous experience in related field
- ability to prioritize duties in a constantly changing environment
- effective management skills
- customer service oriented with patients employees families and other agencies
- good problem solving skills
- excellent time management skills
- ability to accept and give directions
- work well in a team environment
- a desire to make a difference in peoples lives
