Job Description
Work Schedule:
100% day shift. You will work at University Hospital and Eastpark Medical Center in Madison WI.

As an Advanced Practice Provider youll play an integral role in our inpatient and ambulatory care teams by providing highquality costeffective care to patients.
We are seeking a Nurse Practitioner or Physician Assistant to:
- Work collaboratively within a multidisciplinary health team to provide comprehensive care to cancer patients in active treatment or active surveillance in a primarily outpatient setting.
- Provide follow up and urgent appointments at University Hospital and Eastpark Medical Center.
- Perform routine clinical activities independently as outlined in practice agreements/scope of practice including but not limited to the following: clearance assessment for cancer treatment; symptom assessment evaluation and management; assessing cancer patients on clinical trials; as well as obtain chemotherapy signing privileges.

Education:
Minimum
If Nurse Practitioner:
Masters or Doctorates degree from accredited Nursing Program.
If Physician Assistant:
Bachelors or Masters degree from accredited PA Program.
Licenses and Certifications:
Minimum
If Nurse Practitioner:Active RN and APNP license in State of Wisconsin ANDactive board certification as a NP in Family Pediatric Acute Care or Pediatric Primary Care.
If Physician Assistant:Active PA license in State of Wisconsin ANDactive PA board certification.
All Nurse Practitioner and Physician Assistant applicants must have:Active DEA license or ability to apply for such license prior to or upon hire ANDCPR certification and advanced life support certifications if relevant to practice area (ACLS PALS NRP).
