About the role:
The Adult Care Management Team are divided into two; the first manages longerterm complex caseloads with a high emphasis on RelationshipBased Interventions. The second manages highintensity and quick turnaround work under a Discharge to Assess Model. The ethos being to support Adults to leave Hospital when safe and appropriate to do so and continuing their care and assessment out of an Acute setting. Both Adult Care Management Teams hold statutory responsibilities for Safeguarding Reviews and Support Planning. This role sits in Team 2 which follows the Discharge to Assess model. However all successful applicants are likely to receive a hybrid caseload of both type of work.
The Adult Care Management Team work with adults aged 18 and over to identify and care plan for eligible Social Care and Support needs. As a Social Care Assistant within this remit your role will be to manage your own complex caseload with the support of a Practice Manager and Team Manager.

To apply you should have at least 2 years experience in a similar statutory role or be able to demonstrate transferable skills to meet the needs of this this post. You will need to have a current registration with Social Work England and provide your Registration Number. We anticipate that an applicant will have the following aptitudes and ability to demonstrate these:
